ABSTRACT Background: Metallosis is a syndrome of metal-induced synovitis with infiltration and accumulation metallic debris into the periprosthetic structures, including soft bony tissues. The causes chronic inflammatory reaction due to joint instability, pain, osteolysis, implant loosening, or failure. absence specific sign symptom that indicates metallosis difficulty diagnose.Presentation Case: A 35-year-old female, history primary left total hip replacement since seven years ago after avascular necrosis femoral head, presented an eight-month pain limited range motion. Radiograph results showed there no evidence fracture infection. patient underwent revision replacement, which revealed extensive necrotic black metal throughout space.Discussion: Effective treatment requires remove debris, bone graft area address mechanical greatest possible during debridement vital avoid further osteolysis prevent more damage. Excellent clinical result was evaluated by Harris’s score in 18 months postoperatively.Conclusion: This case rare example presenting following replacement.
